We're seeking a dedicated and skilled Advanced Practitioner to join our integrated Care Team, working primarily with patients living with chronic kidney disease and related conditions. You'll be a vital part of a team that prioritizes patient-centered care and focuses on improving health outcomes.
Your Opportunity to Shine:
Provide Personalized Care: Deliver compassionate medical care to patients with chronic conditions, both in their homes and virtually.
Empower Patients: Counsel and educate patients and their families about available resources and strategies for healthier living.
Comprehensive Assessments: Conduct thorough annual wellness exams and ongoing assessments to monitor patient health.
Collaborate with a Team: Work closely with Registered Nurses, Social Workers, and other healthcare professionals to provide integrated care.
Coordinate Care: Liaise with primary care providers, specialists, and ancillary services to ensure seamless care coordination.
Accurate Documentation: Maintain accurate and up-to-date patient records.
Improve Quality of Life: Deliver evidence-based care that reduces hospitalizations and enhances patient satisfaction.
Prescribe and Order: Prescribe medications, order necessary tests, and collaborate with physicians.
Educate and Guide: Educate patients about preventive monitoring and facilitate access to educational resources.
Participate in Team Meetings: Contribute to integrated care team meetings and share your expertise.
Develop and Implement Plans: Develop and implement treatment plans, follow up, and evaluate patient progress.
Adapt and Grow: Take on other duties as assigned and continuously expand your skills.
What You'll Bring:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Access to reliable transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management and medical records systems.
Excellent verbal and written communication skills.
Understanding of federal and state regulations, including HIPAA and OSHA.
2+ years of direct patient care experience.
Managed Care/IPA/Health Plan experience is a plus.
Experience conducting annual wellness visits or similar comprehensive visits.
Bilingual (English/Spanish) highly preferred.
