Job Title: Registered Nurse (RN) – Wound Care

Location: Bronx, NY 10467

Job Summary:  The Wound Care RN conducts health assessment interviews with a focus on wound, ostomy, and continence-related care. This role involves performing focused physical assessments, developing individualized plans of care, and recommending medications and treatments as appropriate. The nurse will collaborate with the healthcare team, educate patients, families, and staff, and actively participate in quality improvement initiatives.

Key Responsibilities:

  • Develop, revise, and evaluate standards of wound care; assist in the selection of supplies, technology, and equipment.

  • Assess and document patient responses to treatment for wounds, ostomies, continence issues, fistulas, and complex wounds.

  • Evaluate the quality and delivery of wound care provided by clinical staff on the unit.

  • Collaborate with the healthcare team and make referrals to other disciplines as needed.

  • Provide patient, family, and staff education to prevent complications related to wounds, ostomies, and continence problems.

  • Develop, deliver, and evaluate staff education programs, including orientation, in-services, and continuing education.

  • Perform routine comprehensive assessments for patients at risk for pressure injuries and initiate preventative measures.

  • Escalate and communicate identified challenges, including ethical issues and conflict resolution.

  • Work closely with the Assistant Director of Nursing for Wound Care to initiate and recommend best practices.

  • Participate in unit, departmental, and hospital-wide quality improvement activities, such as incidence and prevalence studies.

  • Provide culturally competent education and care consistent with patients’ cultural, ethnic, religious, and belief systems.

  • Maintain confidentiality and comply with patient rights policies.

  • Engage in ongoing professional development to maintain competency in clinical practice.

  • Identify and connect patients with community resources as appropriate.

  • Manage assets related to rented equipment (e.g., specialty beds, negative pressure devices).

Required Skills & Experience:

  • Minimum of two (2) years of Medical/Surgical nursing experience.

  • Experience in wound and colostomy care.

Education Requirements:

  • Associate’s Degree (ASN, ADN) in Nursing from an accredited program.

Licensure & Certifications (Required):

  • Active Registered Nurse (RN) License in New York State.

  • Primary Source Verification of license and registration from the NYS Department of Education (NYSED).

  • American Heart Association (AHA) Basic Life Support (BLS).

  • Current certification in one of the following:

    • Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB

    • Certified Wound Ostomy Nurse (CWON) – WOCNCB

    • Certified Wound Care Nurse (CWCN) – WOCNCB

    • Certified Wound Specialist (CWS) – ABWM

    • Wound Care Certified (WCC) – NAWCO
