Job Title: Remote Social Work Case Manager

Hourly Pay: $25 - $45/hour

Key Responsibilities:

• Develop case plans based on clients' social, emotional, and healthcare needs
• Conduct assessments to identify challenges and help clients access resources
• Collaborate with healthcare providers, social services, and community organizations to arrange services

• Provide ongoing support and adjust care plans as necessary

• Assist clients with overcoming barriers, such as financial, housing, or healthcare challenges

• Advocate for clients in legal, healthcare, or educational settings

• Provide crisis intervention and connect clients to urgent resources

• Maintain detailed and confidential records in compliance with regulations

Qualifications:

• Bachelor’s degree in Social Work (BSW) required; MSW preferred

• 3+ years of experience in social work or case management

• Strong understanding of social work principles, ethics, and confidentiality standards
• Excellent communication, problem-solving, and time-management skills

• Ability to work independently and collaboratively within a team

• Experience with healthcare systems, community resources, and legal advocacy
• State licensure as a Social Worker (LCSW or equivalent) preferred
