About the Role
As an essential member of our team, you will provide high-quality care to patients in both in-home and virtual settings. This position is ideal for healthcare professionals who value patient-centered care and want to be part of an innovative approach to chronic disease management.

Key Responsibilities

Conduct comprehensive assessments, including annual wellness exams, in-home and virtual settings.

Educate and counsel patients and families about available healthcare benefits and programs.

Maintain accurate and detailed medical records, including chief complaints, diagnoses, history, medications, allergies, and treatment plans.

Coordinate care with primary care providers, specialists, and ancillary services to ensure comprehensive treatment.

Complete all required documentation in a timely manner.

Maintain high standards of patient care and adhere to evidence-based medical practices.

Prescribe medications, order necessary tests, and collaborate with physicians to optimize patient care.

Promote preventive healthcare by educating patients on wellness initiatives and scheduling necessary screenings.

Participate in integrated care team meetings to enhance collaborative patient care.

Utilize technology and medical data management systems to maintain efficient workflows and communication.

Ensure compliance with federal and state healthcare regulations, including HIPAA and OSHA.

Other related duties as assigned.

What We're Looking For

Bilingual proficiency in English and Spanish (preferred).

Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license.

Board certification (NP: ANCC/AANP; PA: NCCPA).

Current and unrestricted DEA certification.

Ability to work autonomously and make sound clinical decisions.

Proficiency with medical instruments, data management programs, and electronic health records.

Strong communication skills for engaging with patients, families, physicians, and healthcare partners.

A deep understanding of chronic disease management and preventative care strategies.

Minimum of two years of direct patient care experience.

Experience in Managed Care, IPA, or Health Plan settings.

Prior experience conducting annual wellness visits or similar comprehensive assessments in-home or virtually.
