Job Description

Empowered Staffing is partnered with an owner and operator of senior living facilities with communities throughout Indiana, Illinois, and Iowa. The company is currently seeking an experienced MDS Nursing Coordinator to join its facility in Moline to oversee the assessment and care planning process for its residents.


The MDS Nursing Coordinator is responsible for ensuring the accurate, thorough, and timely completion of all Minimum Data Set (MDS) assessments to optimize reimbursement and ensure high-quality resident care. This role integrates assessments across nursing, dietary, social services, therapy, and physician services, ensuring compliance with federal and state regulations while supporting the interdisciplinary team in delivering person-centered care.

Responsibilities

  • Complete comprehensive assessments of all new admissions to determine health status and appropriate level of care.
  • Coordinate and oversee interdisciplinary participation in MDS assessments, ensuring timely and accurate completion of Medicare PPS, quarterly, annual, and significant change assessments.
  • Communicate the level of care and assessment outcomes effectively to all relevant disciplines to guide care planning.
  • Maintain and manage an accurate MDS assessment schedule, ensuring proper reference dates are tracked and adhered to throughout each resident’s stay.
  • Conduct data entry and electronic submission of MDS assessments; verify submissions and perform corrections as needed while maintaining compliance records.
  • Schedule and facilitate resident care conferences in accordance with regulatory requirements, ensuring MDS reviews are completed beforehand.
  • Assist in developing, updating, and evaluating individualized resident care plans to ensure goals are realistic, prioritized, and reflective of residents’ needs and desired outcomes.
  • Monitor and review MDS data to ensure the facility achieves optimal Resource Utilization Group (RUG) categories for accurate reimbursement.
  • Ensure thorough and complete documentation in alignment with federal, state, and facility standards.
  • Manage level of care changes within the facility, notify relevant departments, and submit required forms to appropriate agencies in compliance with state laws.
  • Serve as a resource to nursing staff and interdisciplinary team members on MDS processes, assessments, and care planning requirements.
  • Participate in quality improvement initiatives to enhance resident care and facility outcomes.
  • Perform other duties as assigned to support the clinical and operational goals of the facility.

Qualifications

  • Current and unencumbered Registered Nurse (RN) license in Illinois.
  • Minimum of 1 year of experience in long-term care preferred; prior MDS coordination experience strongly preferred.
  • Thorough understanding of MDS 3.0, Medicare PPS, and state/federal regulatory requirements for reimbursement.
  • Strong assessment, documentation, and care planning skills.
  • Excellent organizational, communication, and interdisciplinary collaboration abilities.
  • Proficient with electronic health record (EHR) systems and MDS submission processes.
