We’re seeking an MDS Coordinat or to help develop individualized care plans and maximize reimbursement — all while upholding the highest standards of care.
Key Responsibilities:
Develop care plans based on MDS assessments in collaboration with the interdisciplinary team
Ensure the facility is compliant with all regulations and guidelines related to MDS
Communicate effectively with staff, families, and healthcare providers to coordinate care
Maximize reimbursement by accurately reflecting care needs in the MDS
Update MDS assessments regularly to reflect health or condition changes
Contribute to overall quality assurance by assuring completeness and accuracy of MDS documents
Gather information through direct observation and collaboration with care team members
The Ideal Candidate :
Licensed Registered Nurse (RN) required
RAC-CT (Resident Assessment Coordinator - Certified) certification required
Experience with MDS and care plan development
Strong communication and organizational skills
Detail-oriented and comfortable navigating regulations
Collaborative team player who advocates for resident care
Familiar with reimbursement processes and compliance requirements
