Job Description

FUNCTION:The RN MDS Coordinator coordinates and assists with completion and submission of accurate andtimely interdisciplinary MDS Assessments, CAAs, and Care Plans according to CMS RAI ManualRegulations and in accordance with all applicable laws, regulations.REPORTS TO:AdministratorQUALIFICATIONS:1. Associate or bachelor s degree in nursing from an accredited college or university2. Currently licensed/registered in state of employment.3. Must maintain an active Registered Nurse (RN) license in good standing throughoutemployment.4. Two (2) years' nursing experience5. CPR certification must remain current throughout employment.6. Advanced knowledge in MDS process7. Read, write, speak, and understand the English language.8. Excellent communication and interpersonal skills.RESPONSIBILITIES:1. The RN MDS Coordinator coordinates and assists with completion and submission of accurateand timely interdisciplinary MDS Assessments, CAAs, and Care Plans according to CMS RAIManual Regulations and in accordance with all applicable laws, and regulations.2. Perform proficiently in all competency areas including but not limited to patient rights, andsafety and sanitation.3. Maintains professional working relationships with all associates, vendors, etc4. Maintains confidentiality of all proprietary and/or confidential information5. Understand and follow company policies including harassment and compliance procedures.6. Displays integrity and professionalism by adhering to the Code of Conduct and completesmandatory Code of Conduct and other appropriate compliance training 7. Report any changes in a patient's condition identified by the MDS Assessment to the DON, UnitManager, and or staff nurse8. Provide education to direct care associates regarding updates or changes to the CMS RAIManual or Skilled Nursing Facility Regulations that impact documentation.9. Assist with review of the Interdisciplinary Comprehensive Care Plan10. Review Final Validation Reports and attest that all assessments have been completed andaccepted into the CMS iQIES system prior to billing and notify the Business Office whenassessments are not ready to bill11. Review CMS Reports to identify assessments completed or submitted late and develop systemsand processes to prevent reoccurrence.12. Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetingsupon request.13. Exhibit excellent customer service and a positive attitude towards patients.14. Demonstrate dependable, regular attendance in accordance with the Employee Handbook15. Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during workinghours.16. Conduct orientation sessions on the MDS process to all employees involved in theprocess.17. Provide ongoing education for employees involved in MDS documentation.18. Maintain documentation of all orientation/training sessions.19. Serve as a resource person for patient care planning activities.20. Work with accounts receivable personnel to facilitate payment.21. Maintain currency with nursing practices and educational requirements.22. Participate in the quality improvement process of the facility.23. Attend and participate in department/facility meetings, as required.24. Access continuing education opportunities appropriate to responsibilities.25. Concentrate and use reasoning skills and good judgment. 26.
