Job Title: LPN – School Nurse
Duration: 13-week contract (extendable)
Location: Massapequa, NY 11758
Shift: Monday – Friday, 8:00 AM – 3:15 PM
Student Diagnosis: Type 1 Diabetes Mellitus
Monitor the student’s blood glucose levels and respond to hypo-/hyperglycemic episodes as needed
Administer insulin and other prescribed medications according to the care plan
Document and administer medications on a scheduled and consistent basis
Recognize and manage behavioral or physical symptoms related to blood sugar fluctuations
Communicate regularly with parents/guardians and healthcare providers
Ensure emergency medical supplies are available and contact information is up-to-date
Collaborate with educators and school staff to meet the student’s medical and academic needs
Participate in care plan meetings and provide timely health updates
Educate school personnel on the student's condition and emergency response procedures
Active LPN license in New York – Required
Valid CPR certification – Required
Pediatric or school nursing experience, particularly with chronic conditions – Preferred
Strong communication and interpersonal skills
Ability to stay calm and act quickly in emergencies
